RehabCare’s outreach services are packages of support which are planned with an individual and support them to live within and provide access to their local communities.
A focus on the importance of the family role in an individual’s life can be central to the service particularly for children and young people. The practical wisdom gained by the family in the role of caring for the person is respected and included in the planning process and implementation of the service in line with the service users will and preference. This may include future planning for temporary or permanent changes in circumstance.
An environment of open communication is encouraged to enable an individual to benefit from integrated support from both their family and the service.
