RehabCare's community-based resource centres in Carlow assists people with disabilities to achieve their full potential and a better quality of life. Our resource centres provide people with access to a range of supports and activities on a one-to-one basis and in group settings. Activities vary from centre to centre and depending on the interests of the people who attend each service some examples are: art and crafts, drama, horticulture, leisure activities, cookery, independent living skills, sports, dancing, group excursions and many more.

This is a second HUB service opened in the town centre of Cavan in 2016. Supporting a number of service users to engage in the local community with projects and programmes based on person-centred planning aims and goals.

Based at Drumalee, Cavan, the resource centre provides services to 50 service users including part-time supported employment, social integration and community inclusion and independent living training. This is a very active service with ongoing development in New Directions type programmes.

This is a hub service which provides services to a number of people in Cavan. Service provision includes social integration and community inclusion projects and programmes led by New Directions. Service users avail of programmes including independent living training, healthy lifestyle programmes, sport leisure activities. Some service users are in part-time supported employment positions with ongoing support from the service as part of New Directions project. Social integration and community inclusion activities form an ongoing part of supports provided at the centre on a daily and weekly basis.

This centre has capacity for 36 people in total. It houses a meeting room, relaxation room, a kitchen/dining area all downstairs. The programmes on offer are meditation, ladies group, men’s group, bingo, quizzes, current affairs, communication class, go for life, one-to-one cooking classes, computers, monopoly, board games walks, general knowledge, pool, library, skittles, arts and crafts, service user activity choice and games. Service users get the chance to gain independence by participating in programme along with partaking in the courses run by the VEC and Rehab Group's learning division, National Learning Network. The service users can go on to college level after the VEC and NLN training if they so wish.

The day centre in Bantry is open Tuesday to Friday 9am -5pm to male and female clients with a physical and sensory disability aged between 18 to 65 at the time of application. We deliver an outreach service to clients in the Castletownbere area on Mondays. Door to door transport is provided to clients who attend the Physical and Sensory Day Centre. The day service offers a wide range of activities within the day service and local community such as yoga, shiatsu, reflexology, art, computers, drama and music, educational courses with local education centres, volunteer work and links in with employability.

Bantry Respite Service aims to provide a break for service users, in a home from home setting whilst providing professional care and support to those using the service. Bantry Respite Service provides a three night respite service (Monday – Thursday or Tuesday - Friday) to any persons both male and female with a physical and/or sensory disability who live in West, North Cork & South Lee Care Area and are between the ages of 18 & 65 years old, at the time of referral. This service is not available for children (under 18 years old) or those who are referred following their 66th birthday.

Servicing the Cork City and county area RehabCare Douglas offers services across a wide range of activities and programmes tailored to meet the needs and wishes of service users including education, social integration and independent living skills programmes.

RehabCare's community-based resource centres in Cork assists people with disabilities to achieve their full potential and a better quality of life. Our resource centres provide people with access to a range of supports and activities on a one-to-one basis and in group settings. Activities vary from centre to centre and depending on the interests of the people who attend each service some examples are: art and crafts, drama, horticulture, leisure activities, cookery, independent living skills, sports, dancing, group excursions and many more.

The service is open 8.30am -5.15pm Monday to Friday and operates a drop in one evening a month and supports adults with a disability aged between 18-65 years. The service operates a variety of activities both centre and community based with options of two or three activities morning and afternoon timetable is available upon request. Activities are reviewed and change according to clients requests

The service links in with local charity shops for volunteer work and other establishments for work experience, employability for employment opportunities and local colleges and schools for further education courses and is part of the social farming programme in West Cork.

Seaview House in Mountcharles provides critical respite services to a number of families in South Donegal. This high-quality nurse-led respite service operates five days a week providing welcome respite for families. A person-centred quality service is guaranteed for the adults and children accessing this service.

This resource centre, located in Lifford caters for more than 30 people spanning a wide variety of ages and backgrounds which makes it a lively, busy and fun environment. The centre operates a programme of different activities and modules based on people’s expressed wishes or desired outcomes. In addition there is a significant focus on one to one work in order to ensure that each person has a chance to explore fully the areas that they would like support and development with.

New Horizons is a resource centre for school leavers with physical, sensory and intellectual disabilities. We are located on the outskirts of Stillorgan village in south Dublin. New horizons is a space for socialising, learning and having fun. We follow the New Directions model by actively promoting community inclusion and offering a high quality individualised service. Our team is also trained in methods of non-verbal communication such as Lamh, PECS and Assistive Technology. We offer both group and individualised programmes based on service users interests, wishes and goals.

Park House is a day service that caters for people with physical disability. Our centre is located close to Stillorgan village in south Dublin. We provide a person-centred approach to create a warm and welcoming environment for all. Park House promotes the enhancement of a service users' quality of life through social and leisure activities such as Art and crafts, photography, ceramics, bridge classes, computers and many more. Park house provides a space for friendly relaxed socialising as well as many opportunities for community inclusion.

RehabCare Ballyfermot is a day resource centre that provides activities and programmes to promote community integration and opportunities for people with disabilities. We promote choice and individual empowerment as well as good health and overall physical and mental well-being. We provide a social space to support people to learn new skills and to access the workforce where required in an atmosphere of camaraderie and friendship.

HOPS is a RehabCare Resource centre that provides supports for people who experience mental health difficulties. It is a centre of wellness and recovery where people can provide support for one another and build new relationships. We provide a variety of activities and wellness programmes.Our mission is to support people to be in charge of their health and wellness, and to be included in their communities.

Staff in HOPS work collaboratively to impart skills, create awareness and meet social needs. We promote open positive relationships, trust and fun. Through the recovery model we endeavour to meet individual needs- physical, mental, emotional and spiritual - in consultation with all stakeholders. We seek to empower others and to reduce stigma. People interested in attending HOPS can ask a GP, a community psychiatric nurse or Psychiatrist to make a referral on their behalf.

RehabCare Dun Laoghaire Outreach Service provides social care services for people over 18 years of age with mild intellectual disability, who may also present with other support needs. The Service is delivered primarily in the community with some independent living skill facilitation. We strive to provide a level of support that enables service users to engage in their own lives as independently as possible. Key worker support is available at regular times each week. The focus is on supporting people to develop coping skills, social connections and achieve Person centred outcomes that enhance their quality of life.

RehabCare Dun Laoghaire Resource Centre provides social care services for 33 people over 18 years of age with intellectual disability. Service users may also present with other support needs due to, for example, mental health issues or Acquired Brain Injury. This is a person-centred community based service that focuses on developing coping skills, social skills and independent living skills. Service Users are supported to engage in meaningful activities in the resource centre and in the community in a way that enhances their quality of life.

Rehabcare provides a bespoke 24 hours acquired brain injury residential service within an individuals own family home. This enables the individual to remain in familiar surroundings, supported by family, friends and the wider community while receiving rehab from specialist social care workers.

The Galway Acquired Brain Injury Service is a neuro outreach service providing individuals with MS or those who suffered a traumatic or acquired brain injury and/or other neurological conditions. Individuals are guided through our various rehab programmes, some of which are ABI management and awareness, personal and behavioural development with life skills management, individually goal focused plans, delivered by ABI specialised community support workers out in the community. The Rehabcare Outreach Service is overseen by a Neuropsychologist and a Neuro Occupational Therapist.

This is an outreach day service for school leavers who have been diagnosed ASD or sensory and physical disabilities. The service provides one-to-one community support and group workshops. The service supports individual's on a one-to-one basis with personal living skills and community/social supports. Workshops cater to a wide range of needs including; nutrition and health, budgeting skills and cyber bullying.

This an individual service-user service which is community based. The service supports the individual with community and social skills and personal living skills. Personal living skills include (but not limited to) budgeting skills, support with grocery shopping management and in-house supports.

Rise service is a community service which provides supports for individuals with an Intellectual Disability, Autism and/or a physical or sensory disability to be more connected, active and included in their community.

Individuals needing additional support following the Compass programme are directed to our Community Outreach Support service in Galway.

Casla Resource Centre is a bilingual centre offering personalised service in line with the HSE's New Directions policy. Our adult service provides individuals to work towards a meaningful and valued life in their community.

This resource centre provides a variety of programmes to service users which support independent living skills, community inclusion and supporting mental health. Programme choices depend on the interests of individuals. FETAC Courses and Educational training programmes are also offered. Those who avail of the service are referred through the HSE, are aged 18 and over, and present with mild to moderate intellectual disability. The aim of the service is to empower those who use it to reach their potential in a warm, welcoming environment. Staff support service users through person-centred planning. The service encourages and supports healthy lifestyles, advocacy, employment, volunteering and active citizenship.

RehabCare Kildare Resource Centre is situated in the heart of Kildare and delivers a person-centred approach in ways that match the choices, aspirations and needs of people with mental health issues, physical and learning disabilities. The aim of the service is to promote the enhancement of service user’s quality of lives through increasing community inclusion, personal development, health, educational, recreational and occupational programmes within the centre and local community.

Many of the service users are supported in areas such as work/employment, independent living, social skills and relationships, education, training and many more. Each person will have the opportunity to access a ‘one of a kind’ service that they have created for themselves

RehabCare Portlaoise Resource Centre is located in the heart of Portlaoise town. The service also extends to the provision of Community Hubs in Laois including Portlaoise, Mountmellick, Potarlington and Abbeyleix which cater primarily for individuals with mild to moderate intellectual disabilities or for individuals with dual diagnosis. The focus of this centre is to facilitate service users to participate meaningfully in their own communities in line with the HSE's New Directions policy by providing person-centred support in social, community, independent living, education, employment and personal development activities.

We also run an Outreach service which provides support to individuals outside the hours of 9am-5pm. This is an evening /weekend service depending on the needs of our service users and is very much social inclusion focused. We run a variety of programmes to met the individual needs of each person who attend the service.

RehabCare's centre-based respite care services provide adults and children with disabilities with the opportunity to take a holiday, while at the same time providing the families/carers with a break from full-time caring.

The presence of a respite service is key in helping a person to remain in the family home for as long as possible. The philosophy of our centre-based services is to provide a home-from-home setting, making it an enjoyable and relaxed time for the individual in an environment where the family can be confident that their family member's needs are being well catered for.

RehabCare Ballinamore is a day service which is person-centred focused with emphasis on the New Directions policy as set out by HSE. The centre supports 37 adults with learning difficulties, mental health issue and physical disabilities. The service is divided into two different areas with a day service in Ballinamore town which operates five days a week, a Hub unit based in Carrick On Shannon which operates four days a week. In line with New Directions we also use the opportunity whilst in this Hub unit for community involvement and for maximising independence. Service users are involved in a variety of programmes to suit their individual needs.

Cairdeas is a physical and sensory respite service. The service provides respite breaks to people with a physical and sensory disability and is based on a social model of support, responding to the individual needs of the guests. The service is available to any person, male or female, with a physical and/or sensory disability who resides in the Mid-West Area and is between the ages of 18 and 65 years old at the time of referral. The philosophy of our centre-based services is to provide a home from home setting, making it an enjoyable and relaxed time for the individual in an environment where the family can be confident that their family member's needs are being well catered for. Our service is designed to meet individual needs. We therefore consider people individually and can accommodate people with varying levels of support needs. A respite break may be any length of time but usually a 5 day or 7 day stay depending on the individual’s needs.

Red House is the Children’s Respite Service located in Limerick. The service strives to provide a holistic service, supporting both child and family, in a home-from-home environment. Red House aims to support children with autism, and their families, through the provision of both residential and day respite services. By providing respite, families are empowered to continue to support their family member within their own home and local community. For many families the service will give them a break, their child is well cared for, happy and they are in a position to do activities or social events that otherwise they have to forgo.

Highfield House provides individuals with a safe home from home environment, while supporting a service user based service. A number of service users from the locality are supported by a team off staff who support their identified needs through support and care.

Providing a holistic service, supporting both child and family, in a home from home environment for children, male and female, with Autism, intellectual disability or physical and sensory disabilities. The children, who currently range in age from 6-18yrs, are referred to the service by the Louth HSE Respite Referral Team. The service currently provides respite care for up to 12 children each week. The service follows the social model of service provision with minimal nursing intervention.

New Visions is a Resource Service that provides a person-centre service to young adults between the ages of 18-25. This service focuses on supporting the giant leap from being a young teenager to adulthood in a fun and goal focused environment. Quayside is an adult resource centre within walking distance to the town centre of Dundalk with easy access to public transport. This service offers a wide range of programmes such as IT skills, cooking, crafts and many others. It also offers the service users the opportunity for work experience out in the community. Dundalk House provides a person-centred service for adults with intellectual disabilities open from Monday to Friday with a focus on supporting persons to achieve a greater level of independence. The Meadows Children's Respite Service provides respite and outreach services to children with intellectual disability, autism and physical and sensory disabilities between the ages of six and 18 years in County Meath. The centre also runs a weekend club for young people between 14 and 18 years and 18-21 who are more independent and require social support. This runs on a Saturday from the resource centre in Navan.

Rehab Care Dunboyne Day Resource Centre provides a range of services to people with physical and sensory disabilities in the south of County Meath. Located within walking distance of the town of Dunboyne and based in the Primary Care Centre at Rooske Rd. Dunboyne. Community Integration, community based programmes, complimentary programmes, person centred planning are the core focus of the Dunboyne resource centre.

Our adult service provides individuals to work towards a meaningful and valued life in their community. Our resource centres provide people with access to a range of supports and activities on a one-to-one basis and in group settings. Each person who attends our resource centres is supported to increase their independence and confidence, enabling them to improve their quality of life. This is done through person-centred planning.

This service is provided in conjunction with Charleville Cottage respite service and is a community-based service provided to individuals who live at home and are supported to utilise community facilities through the outreach service. All referrals come through the HSE and an assessment of needs is carried out by the service to determine the capacity of the service to meet the individual needs.

Canal View’ opened in August 2018 on the main street of Tullamore, and service users currently attending are encouraged to access the resources of the town in a flexible and creative way using the New Directions philosophy. Community inclusion, independence, choice and rights are inherent elements of the multiplicity of supports provided. The main objectives of Canal View are to improve the quality of the lives of all the attendees, while increasing their opportunities for true inclusion within society.

Eiscir Meadows is situated on the outskirts of Tullamore town and is focused on promoting a person-centre approach by supporting each person to exercise choice while availing of the local resources within the community. A strong emphasis is placed on assisting all attendees to reach their full potential in an unstructured and enjoyable way using the New Directions philosophy while embracing a culture of positivity.

Tús Nua Resource Centre is a day service for adults with a mild or moderate intellectual disability. We are situated in the heart of Tullamore within walking distance to all the local amenities and good access to public transport. The resource centre follows the New Directions model of service delivery and all activities are tailored to each person's individual need. Activities offered come under the following categories: meaningful social roles, inclusion in local community, personal expression and creativity, vocational training and employment opportunities, health and well being, personal and social development, maximising independence, education and formal learning, transitions and progression, choices and plans, influencing service policy and practice, bridging programmes to vocational training.

Established in 2016, Ross House is a school leaver's service facilitating school leavers to transition to adult life through social and independent living skills. Currently there are 20 individuals availing of this service.

This Outreach service provides support to individuals in a rural setting, aiding them to access services and leisure activities within their local community in keeping with New Directions policy.

Castlerea Resource Centre offers a personalised service in line with the HSE's New Directions policy. Our adult service provides individuals to work towards a meaningful and valued life in their community.

Sligo resource Centre is a day service which runs from Monday to Friday providing opportunities for service users to access and engage in their wider communities with regards community inclusion, personal choice, meaningful activities. The centre offers a person-centred approach and provides internal and external programmes to suit the needs of those who avail of the service. We are currently in line with New Directions.

Carrow Respite Service is based on a social model of support, responding to individual needs of the service users who are referred. In order to plan and ensure the appropriate levels of support, the service users will be supported within the following defined care groups. This will be determined by their primary condition. Carrow House can only provide respite to those individuals with low support requirements and those that have the physical ability to access all areas of the property required to meet their physical needs.

Thurles Resource Centre is a five day-service for adults with ASD, intellectual disability and associated behavioural challenges from the North Tipperary catchment area. The Resource Centre provides service users with opportunities for personal development, independent living, independent choice, experiential and educational programmes, therapeutic supports, work experience, community integration and social and leisure activities. Needs are met through individual, person-centred plans and tailored timetables. Service users are offered one-to-one staffing support, funded through the HSE.

Serving the areas of North Tipperary and Limerick, Nenagh Resource Centre has provided day service supports since 2002. The service provides educational, recreational and social programmes for participants living with mild to profound ID. The staff team, supported by a Community Employment scheme, provide individual supports to its 29 participants under the HSE's New Directions framework. The participants range in age from 18 to 67, and delivery of service is built upon a person centred approach.

Cluain Glas is a Thurles-based Respite Service which provides respite for both male and female service users between the ages of 18 and 65. Cluain Glas places an emphasis on respect, promotes independence, and enables service users to reach their goals and carry out activities that are of interest to them.

In our CareLink services, we support older people, children and people with disabilities to continue living independently in their own homes. We assist with a wide range of tasks – maybe calling into your home on a regular basis to cook dinner, doing grocery shopping or helping you to get up, washed and ready for the day ahead. We also support people to access the community. CareLink’s home support workers support people to achieve the quality of life that they desire. The services provided are flexible and tailored to the specific needs of the individual client.

At the Vita community hub, we offer our service users a range of supports and opportunities which will help them to develop in their lives and achieve their personal goals. Driven by the core principals of New Directions, our service aims to empower service users to become more independent and confident within their own lives and in their local community. Linking with community organisations such as the VEC, ETB and the SIDO, Vita aims to offer our service users a range of courses and activities in their community which will help them to develop valued social roles. We also offer weekend and evening outings each month which helps service users go to events in and around, Waterford City such as concerts, theatre and sporting events.

Anchor is a RehabCare resource centre based in Waterford, supporting and facilitating adults with disabilities (physical, sensory, intellectual, learning) and mental health difficulties. The service has a strong focus on community engagement, with individualised supports focussed on each person’s wishes and needs. Following the New Directions framework, the team enables people who use the service to reach their potential, develop independence, and become active members of their community, at a pace that suits them. Programmes include art, advocacy, health and wellness, independent living skills, sport, educational and employment programmes. It’s a dynamic service with inclusion at the heart of everything we do. At Anchor, our first port of call is the community.

RehabCare Wexford is a resource centre located on the outskirts of Wexford town offering a day service to adults primarily with learning disabilities, but also people with mental health challenges, acquired brain injury and physical and sensory disabilities. Each service user has their own individual, person-centred plan aimed at enhancing their independence and quality of life in line with New Directions. All programmes run in the centre stem from the service users' outcomes and therefore are wide and varied and include multi-sensory therapy, individual cookery programmes, money management, basic electronics, skin care and photography.

This is a high support service developing individual skills, promoting new experiences and positive social engagement and a positive and active role in their local community in line with the HSE's New Directions model.

River's Edge is located in the heart of Arklow, overlooking the scenic Avoca River estuary. The centre caters for adults with ages ranging from 18 to 50, who have a wide range of interests including music and performance, literature, history, local knowledge, sport and art.
