Clare

GREENACRES

Greenacres Lodge is a residential service which operates 365-days per year. The service meets the care and support needs of each adult as identified through their support plans. The service will promote and advocate for independence in all areas and strive to support the adult in this process. Greenacres has ramped access and has its own transport. All residents have access to GPs, pharmacies, chiropody, behaviour support and any other professional which may be required. A sensory room is in place within the service which offers a calm and relaxing environment for residents. We endeavour to provide and support residents with activities both in-house and in the community. Within the service each resident will be involved in their own day-to-day planning of their personal routines, preferred activities and meals, etc.

THE WILLOWS

The Willows Adult Residential Service meets the care and support needs of each adult as identified through assessment. The service promotes and advocates for independence in all areas and strive to support the adult in this process. The Willows aims to support people to remain as independent as possible in all areas of daily living: including personal care, facilitating and implementing community activities, administration of medication and personal finances. 

INIS GROVE

The service supports both the resident and their family, in a home-from-home environment who are referred to the service by the Mid-Western Health Service Executive.

Inis Grove Adult Residential Service provides care in a person-centred way, working with resident’s dreams, wishes, and needs when and where appropriate. We offer a high-support person-centred service while still maintaining a warm and welcoming environment. The service is based on the social model of care, whilst promoting and supporting the residents in the service with all aspects of health, personal and social care. As such we aim to use local amenities and access the local community.

Lime Lodge

Lime Lodge Service provides a residential service in a person-centred homely environment. We encourage the adults who use our services to be independent in everyday living. For areas that require support and assistance for each individual we have fully qualified and trained support staff that will facilitate our residents. All residents are referred by the Health Service Executive (HSE), Cork. Lime Lodge Residential Service is based on a social care model of support, responding to individual needs of all residents that are referred to our services.

Kerry

Listowel Accommodation Service

Listowel Accommodation Service is to provide a homely person-centred environment which supports both our residents and their families in a home-away-from-home environment. We encourage all adults who use our services to be independent in everyday living. For areas that require support and assistance for each individual we have fully qualified and trained support staff who will facilitate our residents. All residents are referred by the Health Service Executive (HSE), Kerry. 

LACCABEG ACCOMMODATION SERVICE 

The aim of the service is to provide a safe environment which is home to the individuals and where they are valued and respected. Person-centred planning promotes resident’s goals and aspirations and residents are encouraged and supported to achieve their potential.


ROSE LODGE (KILCUMMIN) 

Kilcummin Accommodation Service (Rose Lodge) is based on a social care model of support, responding to individual needs of all residents that are referred to our services. Staff in Kilcummin Accommodation Service have backgrounds in various disciplines, enabling them to best support individual needs. Staff are experienced in developing individualised supports in response to these needs, such as visual schedules, social stories and independent living skills plans, to name but a few.

 

WHITELODGE 

All residents at Whitelodge Accommodation Service will go through a structured and planned transition to ensure their move to the house is managed in a proactive, friendly, and supportive manner.


BOULIA 

The objective of Boulia Accommodation Service is to provide a homely person-centred environment which supports both our residents and their families in a home-away-from-home environment. We encourage all adults who use our services to be independent in everyday living. For areas that require support and assistance for each individual we have fully qualified and trained support staff who will facilitate our residents. All residents are referred by the Health Service Executive (HSE), Kerry. Boulia Accommodation Service is based on a social care model of support, responding to individual needs of all residents who are referred to our services.

GORTACOOSH 

Gortacoosh Accommodation Service operates 24-hours a day, 365 days of the year. The objective of the service is to provide a homely, person-centred environment which supports both our service users and their families in a home-away-from-home environment for up to two adults, male or female. We encourage all adults who use our services to be independent in everyday living. For areas that require support and assistance for each individual we have fully qualified and trained support staff who will facilitate our service users. All service users are referred by the Health Service Executive (HSE), Kerry.

 

Limerick

DROMBANNA

Drombanna Residential Service aims to provide high quality residential care for individuals based on a social model of support, responding to the needs of all individuals who are referred to our services while enabling them to live a life that is as fulfilling as possible.

SEOIDIN

Seoidin is the Children’s Residential Service located in Limerick which provides a holistic service, supporting both children and their families, in a home from home environment.  All children are referred to the service by the HSE. Seoidin is based on a social model of support, responding to the individual needs of the children who are referred.

PARNELL PLACE HOUSE 

Parnell Place Residence is a service created for adults who strive for independence and increasing autonomy. The residents are referred to the service by the HSE in Limerick and surrounding areas. Residents are supported to engage in daily life, and work which reflects their expressed choices and needs.  The staff Team in the Parnell Place Residential Service are committed to having an open, homely, inclusive environment which values each individuals’ choices, knowledge, experience, dreams and aspirations.

Tipperary

TUS NUA 

Thurles, Co Tipperary

Tús Nua will provide a residential service 365 days a year for three service users who present with an intellectual disability, autism and associated behavioural needs. All adults who use our service are encouraged to be independent in everyday living. All service users are referred by the HSE through the North Tipperary Assessment of Needs Committee.

CASTLEVIEW 

Castleview provide's a homely person-centred environment which supports both our service users and their families in a home-away-from-home environment. Based on a social care model of support, responding to individual needs of all service users that are referred to our services. 

MARLSTONE MANOR 

Thurles Residential service provides a homely person-centred environment, supporting both service users and families on a full/part time basis in a home-from-home environment. Thurles Residential Services can accommodate two service users presenting with mild-to-moderate intellectual disabilities and/or mental health needs. The service can accommodate two men/women between the ages of 18 and 65. To ensure consistent levels of support and care, the service will facilitate the service users in various aspects of their daily living as laid out in their support plans. These include but are not limited to: medication administration, social activities, community inclusion, spiritual supports and referral to HSE clinical supports as required e.g. psychology, liaison nurse, psychiatrist, social worker.

SAOL NUA 

Saol Nua provides a homely person-centred environment. We encourage the service users who use our services to be independent in everyday living. For areas that require support and assistance we have fully qualified and trained support staff who will facilitate our service users. The service will meet the care and support needs of the service users as identified through their support plans. 

CLUAIN MUILLEAN 

Cluain Muillean Supported Accommodation provides a homely person-centred environment, supporting both service users and families, in a home-from-home environment for a number of adults. All service users are encouraged and supported in living as independently as possible. To support service users in areas where they need assistance we have fully qualified and trained staff who facilitate the service users. All service users are referred by the HSE through the North Tipperary Assessment of Needs Committee.

CASTLEOAKS 

Nenagh Adult Residential Service (Castle Oaks) was established in 2008 to meet the support needs of adults from the North Tipperary area. This is a seven-day service for adults with intellectual disabilities, mental health issues and associated behaviours. Castle Oaks provides service users with opportunities for independent living, independent choice, personal development, and community integration. In line with the philosophy of RehabCare, the residential service is based on flexibility, equality, and choice.

 
KNOCKLOFTY RESIDENTIAL THE SPINNEY & THE GRANARY 

At Knocklofty Residential Services (The Spinney & The Granary) we provide a safe environment which is home to the individual where they are valued and respected. The directions and decisions a person makes about their life is done through a process of person-centred plans in which they are encouraged and supported to achieve their potential through their goals and aspirations.

RAPLA RISE 

Rapla Rise Residential Service is based on a social model of support.  Rapla Rise Residential Service can accommodate service users with a variety of individual needs.  All service users who will be coming to Rapla Rise Residential Service will go through a structured and planned transition plan to ensure their move to the house is managed in a fun, friendly, and supportive manner.

Carrow House 

Carrow Respite Service is based on a social model of support, responding to individual needs of the service users. In order to plan and ensure the appropriate levels of support, the service users will be supported within defined care groups determined by their primary condition. Carrow House can only provide respite to those individuals with low-support requirements and those who have the physical ability to access all areas of the property required to meet their physical needs.
