RESOURCE CENTRES


RehabCare's community-based resource centres assist people with disabilities to achieve their full potential and a better quality of life. Individuals who use the service are aged between 18 to 65 years and have one or a combination of the following disabilities: Intellectual Disability, Mental Health Issue, Physical Disability, Autism, Sensory Disability, Rare Disorders and/or Acquired Brain Injury.

A person centred planning approach is used while supporting service users to develop their individual plan. This approach involves continual listening and identifying the needs of the service user, focusing on what they would like to do now and in the future all in consultation with their family and friends.

RehabCare resource centres provide service users with access to a range of supports and activities on a one-to-one basis and in group settings.

Activities and supports where possible are supported in the community but some take place within the service. Supports and activities are provided in the following areas:

  • Independent Living
  • Health and Well-being
  • Social and Leisure
  • Employment
  • Education and training
  • Personal Development & Relationships

Community Inclusion, independence, choice and rights are inherent elements of the supports provided.

Service users are encouraged to participate in a balanced range of activities reflecting all aspects of their lives. This varies from person to person according to their individual plan. Typically service users will identify the supports they require and activities they would like to engage in through the person centred planning process, while also being encouraged to engage in activities and supports that will provide them with new experiences.
